Sports Physicals for the 2026-2027 school year will be offered at UCHS on Thursday, April 30th from 6-8pm. Cost is $15 per person.

Please print prior to coming: Participation Physical form, concussion, and sudden cartiac arrest paperwork. All of these can be found on the TSSAA website.
